1. Explain Erikson's psychosocial model and Levinson's theory as they relate to adulthood. What is successful aging?

Erikson is a theorist who focused his work on the psychosocial development of individuals throughout their life. He found interest in social change, cultural diversity and psychological crises through life (Berger, 2008, p. 36). According to his model, Erikson believed that individuals go through eight specific stages of development that help one achieve their ego identity (Hooyman, N. R. & Kiyak, H. A., 2011). In each of these stages, an individual has a major task that is to be accomplished as well as a conflict that requires being resolved. Each of these conflicts is a foundation for the successive stages of this model; therefore, the outcome of the crisis presented infers how the individual will advance to the next stage. Erickson’s concept explained how during development individuals proceed through these various stages as they grow and become more capable of dealing with issues and relate with society overall.
The stages of Erikson’s psychosocial model are as follows: Stage I is basic trust vs. mistrust. The goal of this stage is to establish a basic trust of the world via trust of parent(s). Stage II is autonomy vs. shame and doubt. Here the individual is to establish a sense of autonomy and self from the parent and to establish self-control vs. doubt of one’s abilities. Stage III is initiative vs. guilt. Individuals are to establish an initiative within parental limits without feeling guilty about their emotional needs. Industry vs. inferiority is stage IV of Erikson’s model. This stage deals with establishing a sense of industry within the setting of school, learning new skills without feelings of inferiority or fear of failure. Does therapy change when working with older clients? Drawing upon life span developmental psychology, social gerontology, and clinical experience I have developed a transtheoretical framework for thinking about what changes are needed in psychological interventions with older adults: the contextual, cohort-based, maturity, specific challenge model (CCMSC; Knight, 1996). CCMSC is not a specific therapy system but a framework for thinking about the adaptation of any therapy system to work with older adults. In the model, context means that changes in therapy are often related to the social-environmental context of older adults both in the community and more especially within hospital and nursing home settings, rather than to their developmental stage. Cohort differences are based on maturing in a specific historical time period, leading to a focus on generational groups such as Depression-era generation, GI Generation, Baby Boomers, rather than on age groups. The main reason why suicide rates remain high for older adults even though depression rates are low is due to how symptoms of depressions are perceived in older adults. Suicide remains high because symptoms of depression among older adults are mistakenly seen as part of the aging process and also due to the fact that older adults do not seek mental health services to help them with their problems. Thoughts of suicide among older adults usually go unnoticed. Major factors that account for suicide ideation include psychiatric distress, major depression, poor physical health, and poor social support. In an experimental study done by P.J. Raue et al, participants from a large certified home health agency were randomly selected and assessed for psychiatric symptoms. The study used a Structured Clinical Interview for Axis I DSM-IV Disorders and the Hamilton Rating Scale for Depression. They were asked questions regarding suicidal plans and thoughts, reasons for living, and the capacity to carry out suicidal actions. Of the participants, 11.7% showed signs of suicidal ideation with 2 patients requiring emergency care. Some reports indicated that these patients were thinking of suicide via overdose and firearms. If you consider that in the day and age in which we live a man who is retiring at the age of 65 is maybe able to retire, maybe not, and after probably around 40 years of being part of the work force the amount of money he will be able to retire with is less than a third of what he would have retired with in 1940 when the life expectancy was much longer as well. (1) The first national Social Security was implemented in 1883 and the “magic-age” of 65 was determined as the age when a person should be eligible to no longer work and be taken care by his or her government. It is important to address here that the average life expectancy at this time was only 37 years old. So from its inception only about 54% of men and 62% of women even lived to be the age of 65 and now 130 years later the average American lives to be nearly 78 with some people living way past there. She is 79 years-old, and lives with her husband Robert and her sister Lois, whom she takes care of. She is in good physical shape and does everything on her own, but her short term memory is starting to go downhill. She was just diagnosed with COPD not too long ago due to her closet smoking that she thinks no one knows about and she just had a clot removed from her neck vein, but other than that she has no other limitations. If qualified for Medicare, then Medicare Part A which is financed by payroll tax will cover nursing home cost. About two-thirds of Medicaid spending is on long term care, making it the primary source of financing for long term care. Many residents are guaranteed a retirement income. The most common public source is Social Security benefits. Without social security, 40% of older people would be in poverty. This is the most successful program of the American Welfare State. While there I was able to clearly distinguish which resident had a strong support system and who did not. Families provide a more instrumental support whereas friends provide an emotional support. There were many parent-child relationships with the child mainly being women in most cases. Help provided by children in parents’ old age relates to the amount of contact over years if parents are divorced.
